GP Registrar Training at NEMS

Before you can book any training sessions we need you to identify yourself, provide certain information and exhibit your current medical defence certificate. We also have to initiate a CRB Enhanced Disclosure. This latter formality may not be necessary if you have a Disclosure arranged through your practice or PCT, although we would need to see it before deciding. We also make checks at the GMC.

Please read on and respond appropriately. Please note that you will not be allowed to do any training until we have seen your current medical defence membership certificate (originals are essential - photocopies are not acceptable.)

Documentation needed

We need sight of the following original documents. This is partly for our own purposes and partly to obtain a Criminal Records Bureau Enhanced Disclosure. Please contact Kim Beale to arrange to bring these in. The process should only take about ten minutes, pluse the time taken to complete a CRB Disclosure application form - another ten minutes.

If there is any difficulty in providing any of these items, please telephone Peter McKeown, Personnel Manager or Kim Beale, Assistant Personnel Manager.

  • Completed application form - Download Form
  • Passport
  • Professional Indemnity membership certificate
  • Driving Licence
  • Birth Certificate
  • Gas bill, Electricity bill and Water rates bill. These documents must be addressed to you at your current address and be less than three months old. If this presents a problem, please telephone as above.
  • P45 or P60.

If you have any criminal convictions please do not speak or write to anyone except Peter McKeown, Personnel Manager.

CRB Application Forms

In order to ensure that the CRB Disclosure proceeds efficiently we ask you to pay particular attention to the following:

  • Read the instructions leaflet that will accompany the CRB Application form.
  • Use BLACK ink.
  • Section C: Regardless of what the application form and the CRB instructions leaflet say, the CRB require line 20 to be completed by ALL FEMALE applicants unless the title is "Miss".
  • Take care with Section A, item 9 (At Current Address since.) and Section D items 42 and 45. Please give only MONTHS and YEARS. Please ensure that you give a full five-year address history, with no gaps.
  • DO NOT COMPLETE Sections E and F.
  • Complete Section H, items 66, 68 and 69. Please complete item 67 if you have enclosed any additional information, such as an address history on a separate sheet of paper.
  • DO NOT COMPLETE the back page, sections X Y and Z.

DO NOT send the form to the CRB but hand or send it as soon as possible marked "STRICTLY PERSONAL" to:
